a) Provide ICICIdirect Global Invest Account Number which starts with ‘UXXXXXX’, if any
b) Name of the broker and how will they transfer asset/stock (type of transfer- ACATS, DRS, FOP US, DWAC)
c) The name of country from where the asset/stock will be transferred
d) Share the ISIN of the asset/stock to be transferred OR the holding statement
